Client Notes
1. What are Client Notes:
Client Notes are a brief summary of your pet's medical visit, providing key information regarding the visit and a plan for the next visit. They are provided at no charge.
2. Does every patient receive a Client Note?
Client Notes are generally limited to patients receiving an exam & consultation. Alternatively, we may on occasion extend them to patients with involved prescription instructions, or other involved cases as well.
3. How can I view my Client Notes?
Search for your pet by the day of your visit, Within the past 30 days, search for your pet under the day of your visit. When prompted, your last name is your password. (In the case of multiple or hyphenated last names, only use the "first" last name whose initial appears.) For example, if your pet is named Mousey and your last name is Zuniga-Smith, search under the appropriate day of your visit for the link MouseyZ. Click on the file; when prompted, type in the initialed last name only - "Zuniga" - in this example (again, hyphens or secondary last names are not needed).
4. When are Client Notes posted?
Client Notes are posted usually in the evening, on the day of your visit. Remember to refresh your browser if needed. They remain available up to 30 days past your visit. "LAB" indicates your pet's laboratory results are in - please call the office for details!
