Client Notes
1. What are Client Notes:
Client Notes are a brief summary of your pet's medical visit, providing key information regarding the visit and a plan for the next visit. They are provided at no charge.
2. Does every patient receive a Client Note?
Client Notes are generally limited to patients receiving an exam & consultation. Alternatively, we may on occasion extend them to patients with involved prescription instructions, or other involved cases as well.
3. How can I view my Client Notes?
Search for your pet by the day of your visit, within the past 30 days. When prompted, your last name (case-sensitive) is your password. (In the case of multiple or hyphenated last names, only use the last name whose initial appears.) For example, if your last name is Zuniga-Smith, and your pet is named Mousey and you visited our office Monday January 31st, search under Janury 31st for Mousey. (For multiple or hyphenated last names, your password will be only the initialed last name. In this case, it is MouseyZ, so your password is "Zuniga". Right-click on the file and when prompted, type in "Zuniga" as your password. (Again, hyphens or secondary last names are not needed).

4. When are Client Notes posted?
Client Notes are posted usually in the morning after the day of your visit. Remember to refresh your browser if needed. They remain available up to 30 days past your visit. "LAB" indicates your pet's laboratory results are in - please call the office for details!
